• Review program updates, resources, and data to support care management engagement 
• Discuss the prevention, screening, and treatment and disease progression of dementia. 
• Discuss techniques for routine management and communication with dementia patients 
• Identify community resources and appropriate care settings for patients with dementia and their caregivers. 
• Discuss empathy and review a dementia toolkit to support patients with dementia and their caregivers. 
• Discuss case studies with identification of barriers to care and potential solutions. 
• Identify area resources to support patient’s non-medical needs, including home monitoring devices.
